The county, including all of its cities, was designated as age-friendly by the World Health Organization, the first county in the nation to achieve this recognition. 1. Your First Call: Sourcewise (Area Agency on Aging)

If you don’t know where to start, start here. Sourcewise is the designated Area Agency on Aging for Santa Clara County and a nonprofit organization that has been serving seniors since 1973. [5] Their mission is to provide adults and their caregivers the tools and services they need to effectively navigate their health and life options. [6]
Sourcewise offers an array of direct support services for older adults, adults with disabilities, and caregivers throughout Santa Clara County. [7] When you call, a Community Resource Specialist can connect you with programs like Meals on Wheels, Senior Employment Services, the Health Insurance Counseling & Advocacy Program (HICAP), the Caregivers Network, Care Management, and more. [8]
Their Care Management team will work with you to assess your situation and develop an individualized plan of care that fits your social, physical, and economic needs. [9] For those with complex needs, their Enhanced Care Management (ECM) program offers more intensive, ongoing support, with a dedicated care manager who helps coordinate care across different providers and services. 2. Santa Clara County Department of Aging and Adult Services (DAAS)

The County of Santa Clara’s Department of Aging and Adult Services (DAAS) is dedicated to helping older adults, dependent adults, and the disabled live a safe and independent lifestyle. [11] Their goals include protecting older adults from abuse, neglect, and exploitation; keeping older adults safe and healthy in their own homes; and providing nutritious meals to support the health of older adults. [12]
The Seniors’ Agenda, a partnership between the County of Santa Clara, its 15 cities, and over 100 community organizations, is one of the most powerful collaborative networks in the region for senior advocacy and services. [13] The DAAS website also maintains a comprehensive list of resources for older adults, covering everything from transportation and housing to caregiver support and elder abuse prevention. [14]
3. Nutrition & Meal Delivery
Sourcewise Meals on Wheels

For homebound seniors who struggle to shop or cook for themselves, Meals on Wheels through Sourcewise is a lifeline. The program is available to Santa Clara County homebound adults aged 60 and older, delivering nutritious meals right to their doorstep. [15] Weekly deliveries include seven lunch/dinner meals plus seven breakfast meals, along with milk, fresh fruit, bread, and juice. [16] Sourcewise will even loan a microwave to participants who don’t have one. [17]
The program encourages a suggested contribution of $3.00 per day, but will never deny meals to someone unable to contribute. [18] To get started, simply speak with a Sourcewise Meals on Wheels Nutrition Coordinator to complete the initial intake process over the phone. Within 24-48 hours, meals can be delivered to your door as soon as 5-7 days later. [19]
Santa Clara County Senior Nutrition Program

The Santa Clara County Senior Nutrition Program partners with Sourcewise and other organizations to provide home-delivered meals to older adults throughout the county. [20] The program also supports the CHAMPSS initiative (Choosing Healthy Appetizing Meal Plan Solutions for Seniors), which helps promote healthy, active living among seniors through partnerships with local restaurants. [21]
City of San José Senior Nutrition Sites
The City of San José’s Older Adults Health and Wellness (OAHW) Grant Program collaborates with several nonprofits and public agencies to offer a variety of social services to seniors, their families, and the community at Senior Nutrition Program sites throughout the city. [22] There is a suggested $5 contribution for older adults ages 60 and up at these sites. [23]
4. City of San José Older Adults (50+) Programs

Did you know that the City of San José offers free memberships for older adults ages 50 and up? [24] Throughout San José, the Parks, Recreation and Neighborhood Services department offers classes in health and fitness, dance, yoga, arts and crafts, enrichment and education, music, theatre arts, and more. [25] These programs are a wonderful way for seniors to stay active, engaged, and socially connected, all at no cost.
The City also administers the Older Adults Health and Wellness Grant Program, which seeks to reduce social isolation and improve the health and wellness of individuals ages 50 and up by providing quality programs at Senior Nutrition Program sites and additional service locations within San José. 5. Heart of the Valley, Services for Seniors

Heart of the Valley is a nonprofit, all-volunteer organization helping seniors 62 years or older in West Santa Clara Valley by offering services and programs which help them maintain their independence. [27] Their in-home services for seniors 65 and older include shopping, errands, light yard work, handyman services, home projects, paperwork assistance, and home visits. [28] They serve Campbell, Cupertino, Los Gatos, Monte Sereno, Santa Clara, Saratoga, Sunnyvale, and West San Jose. [29]

6. Transportation Options for Seniors
Getting around can be one of the biggest challenges for older adults who no longer drive. Fortunately, several programs serve the San Jose area:
VTA ACCESS Paratransit

VTA ACCESS paratransit service is provided to eligible individuals with disabilities who cannot use conventional accessible bus and light rail transit service due to their physical, visual, or cognitive disabilities. [31] Standard trips are $4 and premium trips are $16. [32] To apply, call the Eligibility Department at (408) 321-2381 to request a Data Card and schedule a phone interview. [33]
R.Y.D.E. (Reach Your Destination Easily)
R.Y.D.E. is a curb-to-curb transportation and local trip planning service for adults 65 and older living in the west valley communities of Campbell, Cupertino, Los Gatos, Monte Sereno, Saratoga, parts of San Jose, and Morgan Hill. [34] Use R.Y.D.E. for appointments, grocery shopping, visits with friends, or trips to area doctors’ offices and hospitals. [35]
SV Hopper
For seniors in Santa Clara, Silicon Valley (SV) Hopper offers discounted rides within a wide service zone for seniors. [36] Check with the City of Santa Clara Senior Center for current details and scheduling.
7. Legal Assistance for Seniors
Senior Adult Legal Assistance (SALA)
SALA is a nonprofit elder law office whose mission is to support older persons in their efforts to live safely, independently, non-institutionalized, and with dignity. [37] SALA provides free legal assistance for adults 60 and older, covering issues including housing (landlord-tenant disputes, subsidized housing), Medicare and insurance disputes, advance health care directives, powers of attorney, and simple wills. [38]
SALA services are available at multiple senior centers throughout the area, including the City of Santa Clara Senior Center on the 1st and 3rd Wednesdays of the month from 10 a.m. to 12 p.m., by appointment only. [39]
HICAP (Health Insurance Counseling & Advocacy Program)
For Medicare questions, HICAP provides free, one-on-one Medicare counseling by appointment, provided by trained counselors through Sourcewise and funded by the California Department of Aging. [40] Sessions are available at the Santa Clara Senior Center on the 2nd and 4th Tuesdays of the month, from 9 a.m. to 12 p.m. [41]
8. Home Safety & Modifications

- Rebuilding Together Silicon Valley (rebuildingtogethersv.org) provides home modifications to improve safety, accessibility, and mobility for lower-income homeowners, serving San Jose, Campbell, Cupertino, Los Gatos, Santa Clara, Saratoga, and surrounding areas. [42]
- Sourcewise Older Adults Home Modification Program also connects seniors with resources like grab bars, ramps, and medical alert systems to reduce the risk of falls and injuries. [43]
Fall prevention is critical: every second of every day, an older adult falls, and falls are a leading cause of injury and death among older adults. [44] Taking proactive steps to modify the home environment can dramatically reduce this risk.
9. Adult Day Services
Live Oak Adult Day Services

For seniors who need structured daytime support, Live Oak Adult Day Services in San Jose offers a day program from 9 a.m. to 3 p.m. each weekday, including a continental breakfast, lunch, and a snack, along with daily exercise, music and art, socialization, and cognitive games, all at a 5-to-1 client-to-staff ratio. [45] Fees are charged on a sliding scale based on monthly income. [46] This program is also a valuable respite option for family caregivers.
